Premium Assistant

We are assisting qualifying patients to access affordable coverage when they are unable to enroll in adequate, affordable coverage through their employer and don’t qualify for assistance from Medicaid or Medicare. The intention for this agreement is to establish your relationship with DUG Health, to define how Lifeline Health will assist you with your health insurance premiums and/or cost-sharing amounts, and the required qualifications for you to maintain this premium and/or cost-sharing assistance EXCEPTION: Patients with coverage under Medicare Part D or Medicare Part C with prescription drug coverage are not eligible to receive premium assistance
